ABSTRACT

Objective To observe the long-term prognosis and analyze the predictive factors of esophageal cancer patients treated with three-dimensional radiotherapy.Methods A total of 373 patients with esophageal squamous carcinoma who received three-dimensional radiotherapy were retrospectively enrolled in this study.Among these,231 cases received three dimensional conformal radiotherapy (3D-CRT) and the other 142 received intensity modulated radiotherapy (IMRT);202 cases received radiotherapy alone,and the other 171 received chemoradiotherapy;249 cases received involved-field irradiation(IFI),and the other 124 received elective nodal irradiation(ENI);60 cases received a total radiation dose of 50-60 Gy,and the other 313 received 60-70 Gy.Kaplan-Meier method was used to calculate the overall survival (OS) and progression-free survival (PFS).The Logrank single factor analysis and Cox multivariate analysis were used to evaluate predictive factors of PFS and OS.Results The 1-,3-,5-year OS and PFS were 69.4%,33.7%,22.9% and 63.8%,32.8%,22.4%,respectively.The median OS and PFS were 22.7 months (95% CI 18.6-25.4 months) and 19.2 months (95% CI 16.7-21.3 months) respectively.Univariate analysis showed that age,gender,tumor location,three-dimensional technology (3D-CRT vs.IMRT),chemotherapy,prophylactic irradiation to lymphatic drainage area and irradiation dose did not influence OS and PFS (P > 0.05).T-stage,N-stage,TNM-stage and gross tumor volume (GTV) were significantly correlated to OS and PFS (x2 =5.836-14.526,P < 0.05).The multivariate analysis showed that N-stage and GTV were independent predictive factors of OS and PFS (x2 =5.345-12.216,P <0.05).The OS and PFS of patients with two fields of lymph node metastases were worse than those with only one lymph node field metastasis (x2 =4.467,4.169,P < 0.05).Conclusions The long-term efficacy for esophageal cancer patients could be significantly improved through 3D-CRT technology.N-stage and tumor volume were independent prognostic factors of OS and DFS.The number of lymph node metastasis field is significantly related to prognosis.

ABSTRACT

Objective: To investigate the efficacy and safety of using pegylated recombinant human granulocyte-colonystimulating factor (PEG-rhG-CSF) in preventing neutropenia in multiple chemotherapy cycles. Methods: A multicenter, prospective, open-label, singlearmstudy was designed. Patients with malignant tumors, such as lung, ovarian, and colorectal cancers, who received multiple cycles of chemotherapy with the prophylactic use of PEG-rhG-CSF for 2-4 consecutive cycles participated in the study. Results: After the prophylactic use of PEG-rhG-CSF, the incidence of grade IV neutropenia decreased from 4.76% (13/273) in the first cycle to 1.83% (5/273), 1.15% (2/174), and 2.08% (2/96) in subsequent cycles. Meanwhile, the incidence of grade III neutropenia decreased from 11.36% (31/ 273) in the first cycle to 6.23% (17/273), 2.87% (5/174), and 3.13% (3/96) in subsequent cycles. The incidence of febrile neutropenia (FN) during the first cycle was 0.73% (2/273). The duration of FN was 2 days in one case and 5 days in another case. FN was not observed during the second, third, or fourth cycle. After the secondary prophylactic use of PEG-rhG-CSF, the incidence of grade IV neutropenia decreased from 25% (7/28) to 3.57% (1/28), 0% (0/28), and 6.67% (1/15) in subsequent cycles. Meanwhile, the incidence of grade III neutropenia decreased from 71.43% (20/28) to 10.71% (3/28), 14.29% (4/28), and 0% (0/15) in subsequent cycles. The proportion of patients who received antibiotic therapy during the entire chemotherapy period was 10.48% (44/420). Conclusion: The application of PEG-rhG-CSF once per chemotherapy cycle can effectively reduce the occurrence of neutropenia in patients under multiple cycles of chemotherapy treatment with good safety.

ABSTRACT

Objective:To investigate the expression and it's clinical significance of Survivin and Bcl-2 in nasal squamous cell carcinoma (SNSCC). Method:The immunohistochemical Envision two step method was used to measure the expression of Survivin and Bcl-2 in 35 cases of SNSCC, 20 cases of normal inferior concha tissues. Result:The expression of Survivin in SNSCC was 88.6%, significantly higher than the normal inferior turbinate mucosal tissue expression of 0% (P<0.01). The expression of Bcl-2 in SNSCC was 71.4%, significantly higher than the normal inferior turbinate mucosal tissue expression of 25% (P<0.01). Expression of Bcl-2 was significantly higher in SNSCC than in normal tissue. The expression of Survivin was positively correlated with Bcl-2 expression (P<0.01). The higher the grade of tumor, the lower expressions of Survivin and Bcl-2. Conclusion:Survivin and Bcl-2 may play an promoting role in the development and progression process of SNSCC. Survivin may become a target spot of SNSCC gene therapy.

ABSTRACT

Objective To investigate the clinicopathological features of central nervous system hemangioblastoma in von Hippel-Lindau syndrome.Methods The data of 10 patients with central nervous system hemangioblastoma in our hospital since 2013 were analyzed retro-spectively,and the clinicopathological features of central nervous system hemangioblastoma in von Hippel-Lindau syndrome were summarized. Results The macroexamination result showed that most tumor lesions were found in the cerebellum and medulla oblongata,with cystic chan-ges,size from 1 cm to 5 cm,the average size was (3.1 ±0.2)cm,clear boundary,intracapsular yellow cyst fluid.The microscopy result showed tumor foci with a rich blood supply,endothelial cell proliferation and hypertrophy in vascular,which arranged in nests or lobulated mesenchymal cells,the cytoplasm of stromal cells was abundant and lightly stained,a rich lipid was seen,with vesicular or foam.Conclusion The von Hippel-Lindau syndrome is usually cystic lesion,the microscopic examination shows tumor foci with rich blood supply,endothelial cell proliferation,hypertrophy and other changes in vascular.

ABSTRACT

OBJECTIVE@#To investigate the clinical diagnosis, treatment and prognosis of carcinoma showing thymus-like differentiation (CASTLE) of thyroid, and to improve the understanding of CASTLE.@*METHOD@#In 6 patients with CASTLE, we performed region VI lymph node dissection, the lesion resection of thyroid lobe and isthmus. After the operation, except one case of 68 years old patient, the rest patients were treated with radiotherapy 60 Gy in anterior cervical region after third week of operation and 5 courses of chemotherapy with cisplatin, each interval of 2 weeks. Insist on the oral Euthyrox.@*RESULT@#All patients were alive without recurrence and metastasis.@*CONCLUSION@#The diagnosis of CASTLE depends on the histology and immunohistochemistry. Operation was the main treatment. CASTLE is a rare type of thyroid carcinoma with slow progress and good prognosis.

ABSTRACT

Objective To investigate the clinical efficacy and safety of the cytokine-induced killer cells (CIK) treated in patients with advanced pancreatic cancer. Methods 74 cases (hospitalized from January 2012 to December 2013) with advanced pancreatic cancer (Ⅲ ~ Ⅳ stage) were selected. The patients were randomly divided into two groups: CIK cells treated group (38 patients) and chemotherapy group (36 cases). Patients in CIK cells group received CIK cell infusion therapy , and patients in chemotherapy group received only gemcitabine. The efficacy and safety of all patients were observed. Results The efficacy rate of CIK cells group was 29.0%, with no statistical significance compared to chemotherapy alone group (16.7%). Disease control rate was 63.2%, with statistical significance compared to the chemotherapy group (38.9%). The PFS was 3 months (95%CI:2.5 ~ 3.5), OS 6.8 months (95%CI:6.1 ~ 7.5). 6-month survival rate 41.7%, with statistically significance compared to chemotherapy group. Conclusion CIK cells treated in patients with advanced pancreatic cancer is feasible and safe , and can improve disease control rate , improve immunity and produce clinical benefit in patients.

ABSTRACT

Objective To investigate the change of S-100B in serum in rats at varions times after exposure to different whole-brain irradiation doses and to explore the characteristics of the S-100B change in early radiation-induced brain injuries. Methods A model of brain radlation-induced injuries was estabhshed in the rat after whole-brain irradiation with a single dose of 2, 10, or 30 Gy of 4 MeV electron. The concentration of S-100B in serum was detected by means of enzyme-linked immunoserbent assay (ELISA) at different time points (1,6,12,24,3 days, 1 week, 1 month) after exposure. Results In terms of measuring time points, the serum concentration in all groups reached the fwst peak 6 hours after irradiation, declined after 12 hours, rose again after 24 hours, and started to decline again until the last time point (1 month), with the exception of the 2 Gy group, which reached the second peak at 3 days after irradiation. Conclusions The S-100B serum concentration was changed in the rats after whale-brain irradiation and these changes are positively related to the irradiation amount. It seems that the higher the dose, the higher the serum concentration and the differences between the dose groups are significant (via multiple regression models). The S-100B may be a valuable serum marker for the brain irradiation injury.

ABSTRACT

Purpose: To study the treatment of metastasis of late digestive tract cancer?Methods:58 patients with late digestive tract cancer were divided into two groups. One group (reference group) of patients 28 cases were underwent chemotherapeutic treatment while another (treatment group) 30 cases were treated with DDP by CHPP. Results:Through DDP by CHPP (treatment group) the treatment show effectiveness as much 63.3% higher than that of reference group as 39.3%. This indicates the fact that there were significant differences between the groups ( P 0.05)Conclusions: DDP with Cisplatin by CHPP on patients with late digestive tract cancer, especially those patients followed by ascites, can evidently enhance the effectiveness of the treatment.
